​Membership Terms and Conditions 

1. Go Girl Physique Ltd/staff cannot accept responsibility or liability for any consequential harm and/or injury that may result from non-disclosure of any pre-existing medical or physical condition.
2. All monies paid to Go Girl Physique Ltd and/or EziPay Limited are NON REFUNDABLE.
3. The blue sign in key needs to be returned within 30 days of membership expiring otherwise this will incur a fee of $30.
4. Memberships are NON TRANSFERABLE.
5. No membership holds.
6. I am aware that this is an until further notice membership (as per Ezi Pay Agreement) and that I have to give Go Girl Physique 20 business days notice before cancelling the contract after the minimum term.
7. Go Girl Physique Body Series programs need to be paid in full before starting and are non-refundable.
8. The first payment includes – weekly membership fee + $10.25  (Admin & Loading Fee)
9. Late Payment – If you do not make any payment due under this Agreement within 3 Business days of the due date then you shall pay to EziPay Limited the amount due plus:
   (a) $6.07 fees charged for missed payment

           (b) All and any further costs of collection of the amount due incurred by EziPay Limited and/or the Facility Owner.

The Facility Owner and/or EziPay Limited may take such steps as it considers necessary for the recovery of any such amount due.
 
You authorise EziPay Limited to direct debit any amount due under this clause in order to bring the account up to date.
